As part of the Couples Ministry "Get It Done" iniative, we are encouraging all members and friends of Covenant to have an Advance Health Care Directive in place.  Click on the following link and you will be taken to the Alabama State Bar Association's website where you can download the Advance Health Care Planning Brochure and the Advance Directive For Health Care forms.
